The Science Behind Enclomiphene

Enclomiphene is the trans-isomer of clomiphene and functions as a selective estrogen receptor modulator (SERM). It acts primarily at the hypothalamus, where it blocks estrogen-mediated negative feedback signaling.

This increases gonadotropin-releasing hormone (GnRH) pulsatility, which in turn stimulates the pituitary gland to release luteinizing hormone (LH) and follicle-stimulating hormone (FSH). LH supports Leydig cell testosterone production, while FSH contributes to spermatogenesis.

Fertility-focused use: Primary goal is maintenance or improvement of sperm parameters via FSH/LH support

TRT-alternative use: Focus is restoration of endogenous testosterone production while preserving hypothalamic–pituitary–gonadal axis activity, avoiding suppression seen with exogenous testosterone
